    what can randomisation or blinding mean in the context of a case series which by definition only has one group?

    I just read the paper; yes they do call it a "Prospective randomized single blinded clinical case series study". It certainly was not randomized with only one group. Weird!

    A very thought provoking subject, but disappointingly, no activity. Let's see if I can motivate some comments:

    Points of fact (agree or disagree)

    1. Orthotics should not be prescribed until a diagnosis is made which necessitates their use.
    2. Use abnormal (hyperpronation, gravity drive pronation, etc.) pronation is a symptom, not a diagnosis.
    3. What is the etiology of the observed abnormal pronation. Needs to be determined before contemplating orthotic intervention.
    4. Pediatric flatfoot is an observation. What is the etiology of the observed Pediatric flatfoot. Needs to be determined before contemplating orthotic intervention.

    What are your thoughts?

    Orthopedics, as a subspecialty in Podiatry, disappeared when the American College of Podiatric Orthopedists closed in the mid 1970s. Since then, the thrust in Podiatric medicine has been surgical.

    I believe, at this point in time, many if not most Podiatrists have no understanding or opinion regarding the above 4 points of fact. I believe
    • Many, if not most Podiatrists, think abnormal pronation is a diagnosis (which it is not).
    • Many Podiatrists have no idea what causes the foot to abnormally pronate.
    • Many Podiatrists think Pediatric flatfoot is a diagnosis (it is an observation, not a diagnosis)
    Just food for thought.
