Correlation between health-related quality of life and venous leg ulcer's severity and characteristics: a cross-sectional study

This study aims to determine health-related quality of life (HRQoL) in patients suffering with venous ulceration and to correlate wound's severity status with HRQoL loss as well as identify the aspects of HRQoL most negatively affected by the presence of venous ulcers. In this observational, cross-sectional, descriptive, analytical multi-centre study, data was compiled over a period of 3?5 months. Thrity-four patients with venous ulceration were recruited. The RESVECH 2?0 scale was used to monitor wounds. The MAID scale was used to measure wound's severity. The Charing Cross Venous Ulcer Questionnaire (CCVUQe) (Spanish version) was used to evaluate quality of life. The mean CCVUQe score was 60?58 ? 16?04. The HRQoL dimension most affected was ?Emotional state? (mean score = 77. 67 ? 17?34). The average RESVECH 2.0 score for the wounds was 13?15 ? 5?07. A statistically significant association between total CCVUQ-e score and total RESVECH 2.0 score was detected [Pearson correlation coefficient r = 0?546 (P ≤ 0?001)]. Venous ulcers affect patients' HRQoL, particularly their emotional status. There is a relationship between the severity of the wound and loss of HRQoL. The presence of non-viable tissue, poor exudate control and infection all determine loss of HRQoL. New studies are needed to confirm these findings.

Background
In the care of people with chronic wounds, nursing includes, besides direct wound care, in particular the safeguarding and promotion of health-related self-management and well-being. Therefore, it is important to assess the health-related quality of life of patients or limitations and impairments in different areas of life to be able to take appropriate measures if necessary. The aim of this study was to assess the subjective impairments experienced by people with chronic wounds in different quality-of-life dimensions.

Material and methods
The study design was a quantitative cross-sectional survey. Data were collected by means of Wound-QoL (questionnaire on quality of life with chronic wounds). In total, 59 patients participated in the study.

Results
According to the results of this study, there is a need for action in all areas of Wound-QoL. Up to 64% of the participants felt moderately to severely impaired in the different QoL dimensions.

Conclusion
Despite the fact that the wound care situation seems to be optimal, the quality of life (QoL) of people with chronic wounds often is impaired. By using Wound-QoL in day-to-day wound care, the care of people with chronic wounds could be optimized on the basis of patient-related outcome factors.

How does a chronic wound change a patient's social life? A European survey on social support and social participation

Chronic wounds can severely limit patient's social life. This cross-sectional study investigated quantitatively social support of patients with chronic wounds, its association with health-related quality of life as well as qualitatively changes in social participation of these patients. Overall, 263 patients from seven countries participated. The most frequent wound class was leg ulcer (49.2%). Results revealed generally high levels of social support (mean global score: 5.5) as measured with the Multidimensional Scale of Perceived Social Support. However, individuals differed considerably (range 1.0–7.0). All dimensions of social support differed by patients' family and living situations (p < 0.001 to p = 0.040) and were positively correlated with generic health-related quality of life (r = 0.136–0.172). Having children, living with others and being in a relationship were significant predictors of having higher global social support. Patients reported great support from family members. Many participants reported no changes in relationships with friends. Wound care managers took an important role and provided additional emotional support. Patients reported a range of discontinued activities. Despite the high overall level of social support, inter-individual differences should be acknowledged. The importance of family carers should be acknowledged to be able to reduce caregiver burden and to ensure high-qualitative wound care.
