Objectives: The purpose of this study was to investigate the effectiveness of conventional radiofrequency (CRF) ablation treatment on chronic plantar heel pain due to heel spur.

Methods: A total of 20 patients with heel spur who did not respond to conservative treatments were recruited for the study. Under fluoroscopy guidance, CRF was performed to three points at the top, above, and below the heel spur in the longitudinal plane of the foot. Pain intensity, the pressure pain threshold (PPT), and functional status were assessed using a visual analog scale (VAS), pressure algometers, and the Foot Function Index (FFI). All measurements were taken before the procedure, as well as 1, 3, and 6 months following the procedure.

Results: CRF was applied to 20 patients - 16 (80%) females and 4 (20%) males. Their mean age was 51.40+-8.10 years, the mean body mass index was 33.80+-5.47 kg/m2, the mean duration of symptoms was 18.30+-9.02 months, and pes planus was present in 5 patients (25%). A statistically significant decrease was observed in VAS score and PPT and FFI measurements at the 1st, 3rd, and 6th month following CRF compared to before CRF (p<0.001).

Conclusion: CRF is an effective, safe, minimally invasive method to reduce pain severity in patients with chronic heel pain due to heel spur in the short (0-3 months) and intermediate term (3-6 months).

Treatment of Plantar Fasciitis in Patients with Calcaneal Spurs: Radiofrequency Thermal Ablation or Extracorporeal Shock Wave Therapy?

Background and objectives: We aimed to compare the effectiveness of ESWT (Extracorporeal Shock Wave Therapy) and RFA (Radiofrequency Thermal Ablation) on pain, disability, and activity limitation in the treatment of plantar fasciitis in patients with calcaneal spurs.

Materials and methods: Patients who apply to Orthopedics and Traumatology and Physical Medicine and Rehabilitation departments with a complaint of heel pain are included in this retrospective study. We included patients diagnosed with calcaneal spurs who received treatment with ESWT (n = 80) and RFA (n = 79) between 1 August 2021 and 1 September 2022. All patients were evaluated using the Visual Analog Scale (VAS), Foot Function Index (FFI), and the Roles and Maudsley score (RM) before and after treatment. An evaluation was performed on average 6 months after treatment.

Results: This study included 79 RFA patients (34 females and 45 males) with a mean age of 55.8 ± 9.6 years and 80 ESWT patients (20 females and 60 males) with a mean age of 49.1 ± 9.5 years. There was a significant decrease in VAS scores after treatment in both the RFA and ESWT groups (z: -4.98, z: -5.18, respectively, p < 0.001). The reductions in FFI pain, FFI activity restriction, FFI disability, and RM scores were significant in both groups, although the scores after treatment were lower in the RFA group.

Conclusions: This study demonstrates that ESWT and RFA significantly reduced pain, disability, and activity restriction in the treatment of plantar fasciitis in patients with calcaneal spurs. ESWT proved particularly effective in alleviating pain, whereas RFA had more pronounced effects on reducing disability and activity limitations. The choice of treatment should be based on the patient's specific complaints.

Abstract: Introduction: Plantar Fasciitis (PF) is one of the most common causes of heel pain, with a prevalence ranging from 3.6-7% in the general population. Approximately 90% of patients with PF respond well to conservative therapy, including rest, stretching, analgesics, physical therapy, shoe modifications, or steroid injections. However, around 10% of patients have recalcitrant cases that require invasive or surgical interventions. Pulsed Radiofrequency Ablation (PRFA) is a non neurodestructive method that provides non surgical pain relief in such cases. Aim: To compare the effectiveness of ultrasound-guided PRFA of the Medial Calcaneal Nerve (MCN) and Extracorporeal Shockwave Therapy (ESWT) in managing recalcitrant PF. Materials and Methods: This randomised controlled trial was conducted at the Department of Physical Medicine and Rehabilitation, Regional Institute of Medical Sciences, Imphal, Manipur, India over a two-year period from September 2018 to August 2020. A total of 78 patients with PF were included in the study, divided equally into two groups: a study group and a control group, each consisting of 39 participants. The study group received PRFA of the MCN, while the control group received ESWT of the plantar fascia. The Visual Analog Scale (VAS), Plantar Fascia Thickness (PFT), and American Orthopaedic Foot and Ankle Society (AOFAS) hindfoot score were measured at 1, 4, 12, and 24 weeks as outcome variables. Data analysis was performed using Statistical Package for the Social Sciences (SPSS) version 21.0, and statistical tests like Chi-square test, t-test, and repeated measures Analysis of Variance (ANOVA) were used. A p-value of <0.05 was considered significant. Results: Of the 78 patients included in the study, 42 (53.8%) were females and 36 (46.2%) were males, with a mean age of 41.08±10.09 years. The VAS score in the study group improved from 5.51±0.82 at baseline to 1.92±0.80 at 24 weeks, while in the control group, the VAS score improved from 5.36±0.97 at baseline to 2.33±0.66 at 24 weeks. PFT in the study group improved from 4.45±0.374 at baseline to 2.26±0.231 at 24 weeks, while in the control group, PFT improved from 4.42±0.366 at baseline to 2.49±0.357 at 24 weeks. The AOFAS score also improved from 50.49±13.13 at baseline to 74.03±11.53 in the study group, and in the control group, the AOFAS score improved from 49.74±12.26 at baseline to 71.36±10.72 at 24 weeks (p-value <0.05). Conclusion: Significant improvements were observed in all outcome measures in both the study and control groups, with a p-value <0.05. However, the improvements were more pronounced in the study group, with minimal to no side-effects. Hence, ultrasound-guided PRFA of the MCN can be considered an effective minimally invasive treatment modality for recalcitrant PF.

A Novel Combination Strategy of Ultrasound-Guided Percutaneous Radiofrequency Ablation and Corticosteroid Injection for Treating Recalcitrant Plantar Fasciitis: A Retrospective Comparison Study

Introduction: The best treatment yielding clinical benefits was still equivocal and controversial for the treatment of recalcitrant plantar fasciitis (PF). This study aimed to propose a novel combination strategy of ultrasound-guided percutaneous radiofrequency ablation (RFA) and corticosteroid injection (CI) for recalcitrant PF, and to compare its therapeutic effects with CI alone and continued conservative management.

Methods: We retrospectively reviewed consecutive patients with recalcitrant PF who underwent combined strategy (RFA + CI), CI alone, and continue conservative treatment at our institution between October 2021 and February 2023. The technical pearls were described elaborately. A comparison of demographic data and clinical outcomes, including visual analog scale (VAS), Ankle-Hindfoot Scale (AOFAS-AHS), and plantar fascia thickness, were conducted among the three groups.

Results: Seventy-one eligible patients were enrolled in this study, with 17 in the combined strategy group, 25 in the CI group, and 29 in the continued conservative treatment group. Both the combined strategy group and the CI group showed significant improvements in VAS scores, AOFAS-AHS scores, and significant reductions in plantar fascia thickness during the 12-month follow-up period compared to those preoperatively (P < 0.05). The combined strategy group achieved comparable immediate pain relief to the CI group after the intervention ([25.7 ± 15.7] vs. [20.6 ± 17.6], P = 0.850). However, the combined strategy group demonstrated superior improvement in symptom and function compared to the CI group at the 3-month (VAS: [21.9 ± 13.5] vs. [39.6 ± 20.4]; AOFAS-AHS: [77.9 ± 12.4] vs. [60.5 ± 17.4], P < 0.05) and 12-month follow-up (VAS: [15.7 ± 12.0] vs. [56.8 ± 17.5]; AOFAS-AHS: [84.5 ± 10.7] vs. [53.8 ± 12.4], P < 0.05). Obvious adverse effects or complications were not identified in either group, while two cases (11.8%) in the combined strategy group and five cases (20.0%) in the CI group experienced unsatisfactory symptom remission.

Conclusions: We introduced and detailed a novel combination strategy involving ultrasound-guided percutaneous RFA and CI for treating recalcitrant PF. The strategy is both effective and safe in alleviating pain and enhancing function throughout the entire treatment course.

Objective: This study is an open clinical trial. The aim of this study was to show the changes that occur in the viscoelastic properties of the plantar fascia (twenty healthy volunteers) measured by SEL and the changes in the plantar fascia temperature measured by thermography after the application of a 448 kHz capacitive resistive monopolar radiofrequency (CRMR) in active healthy subjects immediately after treatment and at the 1-week follow-up.

Methods: Furthermore, to analyze if an intervention with 448 kHz CRMR in the plantar fascia of the dominant lower limb produces a thermal response in the plantar fascia of the non-dominant lower limb. The final objective was to analyze the level of association between the viscoelastic properties of the PF and the temperature before and after the intervention with 448 kHz CRMR.

Results: Our results showed that a temperature change, which was measured by thermography, occurred in the plantar fascia after a single intervention (T0-T1) and at the 1-week follow up (T1-T2).

Conclusion: However, no changes were found in the viscoelastic properties of the plantar fascia after the intervention or at the 1-week follow up. This is the first study to investigate changes in both plantar fascia viscoelastic properties and in plantar fascia temperature after a radiofrequency intervention.

Innovative approaches in the treatment of chronic plantar fasciitis: comparison of pulsed radiofrequency ablation and surgical intervention

Purpose: This study aimed to compare the effectiveness of Pulsed Radiofrequency Ablation (PRFA) and surgery for treating chronic plantar fasciitis, focusing on pain relief and functional outcomes.

Methods: A prospective study involved 30 patients with chronic plantar fasciitis unresponsive to 12 months of conservative treatment. Patients were divided into PRFA (n = 17) and surgical (n = 13) groups. Clinical evaluations were conducted preoperatively and at three, six and 12 months postoperatively using VAS, AOFAS, FFI, and RMS scores. Radiological measurements assessed foot structure impact.

Results: Both PRFA and surgery significantly reduced pain and improved function. PRFA had a shorter operative time and quicker return to activities (p < 0.001). At 3 months, PRFA showed superior VAS, FFI, and RMS scores (p < 0.05). Long-term outcomes were similar. No major complications occurred, but minor complications were higher in the surgical group (p < 0.01).

Conclusions: PRFA is a minimally invasive, effective treatment for chronic plantar fasciitis with quicker recovery and lower complication rates compared to surgery. Both treatments offer comparable long-term benefits. Further studies are needed to confirm these findings.

Background
Plantar fasciitis, a condition marked by persistent and often excruciating heel pain, frequently poses a
formidable hurdle when conservative treatment approaches fall short. This multi-centered retrospective

(RFNA), an innovative and less invasive procedure, as a novel avenue for treating recalcitrant plantar
fasciitis. This investigation centers around a group of 24 patients who have faced the persistence of this
challenging ailment. By meticulously scrutinizing patient outcomes and conducting a comprehensive
analysis of safety aspects, this study aspires to offer enlightening revelations regarding the promise and
practicality of pulsed RFNA as a therapeutic solution for tackling this intricate and tenacious condition.
Methods
This retrospective study included 24 patients who had undergone pulsed RFNA for recalcitrant plantar
fasciitis between June 1, 2020, and June 1, 2022, at Hospital Pengajar Universiti Putra Malaysia (HPUPM),
Hospital Universiti Teknologi Mara (UiTM), and Hospital Serdang. Patients were selected from the
Orthopedic Clinics at HPUPM, Hospital UiTM, and Hospital Serdang and were screened according to the
inclusion and exclusion criteria. Patient data was extracted from the hospital information system and
electronic medical records. Pre-procedure and post-procedure assessments were conducted at one, three,
and six months on the selected patients using the visual analog scale and American Orthopaedic Foot and
Ankle Society Ankle-Hindfoot Scoring systems. All selected patient data was traced and tabulated
accordingly.
Results
This study evaluates the effectiveness of pulsed RFNA in treating recalcitrant plantar fasciitis in 24
participants (39 feet). Results show a significant reduction in pain and improvement in functionality at one,
three, and six months post-RFNA. Demographic factors (age, gender, and specific diagnosis) did not
significantly impact outcomes. The study supports pulsed RFNA as an effective treatment for recalcitrant
plantar fasciitis, emphasizing consistent benefits across various patient characteristics.
Conclusion
In conclusion, the study demonstrates the notable effectiveness of pulsed RFNA in improving pain reduction
and functional outcomes for individuals with recalcitrant plantar fasciitis. The findings, consistent across

Comparison of tibial nerve pulsed radiofrequency and intralesional radiofrequency thermocoagulation in the treatment of painful calcaneal spur and plantar fasciitis: a randomized clinical trial

Objective
Ultrasound-guided tibial nerve pulsed radiofrequency (US-guided TN PRF) and fluoroscopy-guided intralesional radiofrequency thermocoagulation (FL-guided intralesional RFT) adjacent to the painful calcaneal spur are two interventions for pain management in painful calcaneal spur and plantar fasciitis. This study aimed to compare the effectiveness of the two procedures.

Design
A prospective, randomized, single-blind study.

Setting
Single-center pain clinic.

Subjects
Forty-nine patients who met the inclusion criteria were randomized into two groups.

Methods
Group U (25 patients) received US-guided TN PRF at 42°C for 240 s, whereas Group F (24 patients) received FL-guided intralesional RFT at 80°C for 90 s. The most severe numeric rating scale (NRS) score during the first morning steps and the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot scores were used to evaluate the effectiveness of the procedures. The study’s primary outcome assessed treatment effectiveness via the NRS, whereas the secondary outcomes included changes in the AOFAS score and the incidence of procedure-related mild adverse events.

Results
NRS and AOFAS scores significantly improved in Groups U and F at 1 and 3 months compared with baseline (P < .05), and there was no significant difference between the groups. At month 1, 50% or greater pain relief was achieved in 72% of patients in Group U and 75% of patients in Group F. No significant difference was observed in the incidence of mild adverse events between the groups.

Conclusions
US-guided TN PRF and FL-guided intralesional RFT have shown significant effectiveness in the treatment of painful calcaneal spur and plantar fasciitis. Larger randomized controlled trials are needed

Efficacy of Radiofrequency by the Topaz Technique for Chronic Plantar Fasciopathy: Systematic Review and Meta-Analysis

Background/Objectives: Chronic plantar fasciopathy is a degenerative pathology that elicits persistent heel pain, significantly impacting quality of life. When conservative treatments fail to yield satisfactory outcomes, radiofrequency microtenotomy utilizing the Topaz technique presents a minimally invasive alternative with regenerative potential. This study aims to evaluate its efficacy in pain reduction, functional improvement, and complication rate compared to other treatments. Methods: A systematic review was conducted in accordance with the PRISMA guidelines and registered in PROSPERO (CRD4202525648314). PubMed, EBSCOhost, Web of Science, and Scopus (2014-2024) were comprehensively searched to identify studies on the Topaz technique for refractory chronic plantar fasciopathy. Clinical trials, cohort studies, and case series were included, and a meta-analysis was performed using a random-effects model to assess pain, the Visual Analog Scale (VAS), American Orthopaedic Foot & Ankle Society function(AOFAS), and complications. Results: Fifteen studies encompassing 1576 patients were analyzed. The meta-analysis demonstrated a significant reduction in pain of 5.90 points on the VAS scale (95% CI: 5.03 to 6.77, p < 0.001) and a functional improvement of 0.28 points on the AOFAS scale (95% CI: 0.27 to 0.28, p < 0.001). The complication rate was low (3.00%), with high patient satisfaction (90%) and rapid recovery. Conclusions: The findings suggest that the Topaz technique is a safe and effective option for chronic plantar fasciopathy, demonstrating significant improvements and minimal complications.

Comparative results of the use of modified bipolar radiofrequency ablation in patients with proximal plantar fasciopathy

Background. Among the many causes of plantar heel pain, the most common is proximal plantar fasciopathy (PF), second only to ligamentous injuries of the foot. The disease reduces the quality of life and is difficult to treat, as its pathogenesis remains unexplored.

The aim of the study — to conduct a comparative evaluation of treatment outcomes in patients with proximal plantar fasciopathy using extracorporeal shock wave therapy, as well as minimally invasive bipolar radiofrequency ablation, both with and without the described modification.

Methods. We analyzed the treatment outcomes of 36 patients who sought medical care for chronic heel pain caused by proximal PF in the period from 2018 to 2023. Among the patients, there were 14 (38.8%) women and 22 (61.2%) men, with a median age of 55.4 [46.7; 61.7] years. All patients were randomly assigned to three groups of 12 patients each. In Group 1 (control), extracorporeal shock wave therapy (ESWT), which had not been used at previous stages, was used for treatment; in Group 2 (comparison) — minimally invasive bipolar radiofrequency ablation (BRFA); in Group 3 (main) — minimally invasive BRFA using a method modified by the authors. Comparative evaluation of the results was carried out at 1, 3, 6 and 12 months after surgery in Groups 2 and 3 and after the completion of ESWT course in Group 1.

Results. The median plantar fascia thickness of the affected limb did not differ between the groups at 3 months after the completion of treatment. At 6 months, these indicators were significantly different between the control and main groups (p = 0.001). In the intergroup analysis of the dynamics of pain syndrome and foot functionality, the treatment results in the main group showed a statistically significant advantage compared with control and comparison groups after 1, 3 and 6 months of follow-up (p < 0.05).

Conclusions. The modified minimally invasive radiofrequency ablation method for the treatment of patients with proximal plantar fasciopathy demonstrated superior early clinical outcomes compared to the standard ablation technique and a course of extracorporeal shock wave therapy. The results obtained appear promising and suggest that the modified technique may be considered a preferred treatment option in cases where all types of conservative therapy fail within a six-month period.

Abstract
Background
Plantar fasciitis is one of the most common causes of chronic heel pain, accounting for up to 10 % of running-related injuries and significantly impacting the quality of life in both active and sedentary populations. While the majority of patients respond to conservative measures such as stretching, physiotherapy, orthoses, steroid injections, and extracorporeal shockwave therapy, approximately 5–10 % remain symptomatic despite prolonged non-operative treatment. Surgical intervention is considered in these refractory cases, with both open and endoscopic plantar fasciotomy traditionally employed. However, these approaches are associated with potential drawbacks, including wound complications, prolonged recovery, and altered foot biomechanics. In recent years, minimally invasive alternatives have been investigated to minimize morbidity while providing effective pain relief. The study was conducted to evaluate the clinical outcomes of percutaneous plantar fasciotomy using the microdebrider TOPAZ coblation wand in patients with recalcitrant plantar fasciitis.
Methods
A hospital-based, retrospective observational follow-up study in patients with a primary clinical diagnosis of recalcitrant PF, supported radiologically and underwent percutaneous plantar fasciotomy using a microdebrider TOPAZ coblation wand and had a minimum follow-up of 1 year. Outcome was evaluated using the Foot and Ankle Ability Measure (FAAM) Score, Numerical Rating Scale (NRS)
Results
Of the 18 patients included, 16 (88.9 %) were females. Thirteen (72.1 %) were aged 30–50 years. None had anatomical deformity. There was a significant reduction in pain levels from pre-operative NRS scores (mean 7.56) to NRS scores at 1 year (mean 2.83) (p < 0.001).
Functional ability improved notably from a pre-operative FAAM score (mean 59.94) to a 1-year FAAM score (mean 91.33) (p < 0.001)
Conclusion
Radiofrequency coblation using TOPAZ microdebrider wand is an effective intervention for chronic recalcitrant PF.
