Kinematics analysis of subtalar eversion during gait in women with fibromyalgia

Objective
To analyze the subtalar eversion range of motion during walking in women with fibromyalgia.

Method
Twenty women diagnosed with fibromyalgia were directed to walk barefoot at comfortable and self-paced speed on a 7-meter walkway. Subtalar eversion range of motion was measured by difference between the maximum and minimum values of subtalar eversion in stance phase. It was considered as reference values for subtalar eversion during gait a range of motion between 4° e 6°. Descriptive statistics were performed.

Results
In both right and left lower limb analysis of subtalar eversion range of motion, five women showed joint hypomobility, and twelve showed hypermobility. Only one patient performed unaltered subtalar eversion range of motion in the both lower limbs. Both joints expressed high variability, and were not identified significant differences between the right and left sides.

Conclusions
Our findings suggest that biomechanical function of the subtalar eversion during loading response phase of gait in women with fibromyalgia, by excessive rigidity or complacency joint, tends to be impaired. This finding suggests that the indication of walking as an auxiliary strategy in the treatment of people with fibromyalgia should be preceded by thorough examination of the mechanical conditions of joint of the patient.

Background
Fibromyalgia is characterized by generalized pain. To our knowledge, there are no previous reports describing the specific podiatric characteristics of patients with fibromyalgia.

Objectives
To describe podiatric characteristics of patients with fibromyalgia.

Material and Methods
Case-control study including 50 patients with fibromyalgia and 50 sex and age-matched controls. After obtaining written informed consent for the study, we collected demographic data and also measured pain in the leg, ankle and foot area with a visual analogue scale (VAS) and performed a general physical examination to test mobility and rigidity of the foot joints and also asked the patients about their shoeing habits.

Results
Our results confirm that patients with fibromyalgia experience significantly more pain in all the leg, ankle and foot areas than control patients. For this reason they employ significantly more pain-killing drugs than controls. However, this increase in pain is not associated to stiffness or mobility abnormalities in our series. Besides, patients with fibromyalgia show no increase in the rate of hyperkeratosis or other podiatric lesions.

Conclusions
Patients with fibromyalgia do not seem to show a higher rate of podiatric lesions or a peculiar pattern of foot involvement compared to age and sex-matched controls.

Quality of life related to foot health status in women

Introduction: To date, the Foot Health Status Questionnaire (FHSQ) has not
been applied to women who suffer from fibromyalgia. The main purpose of
this study was to compare both foot and general health-related quality of
life between women with fibromyalgia and healthy matched women. We hypothesized
that women with fibromyalgia may present an impaired quality
of life related to foot and general health.
Material and methods: A sample of 208 women, mean age of 55.00 ±8.25
years, was recruited from an outpatient clinic and divided into 2 groups,
104 women with fibromyalgia (for the case group) and 104 healthy matched
women (for the control group). Demographic data and the domains of the
FHSQ scores were registered.
Results: Statistically significant differences (p < 0.001) between case and
control groups were found for both all specific foot domains (pain, foot function,
foot health and footwear) and all general wellbeing domains (general
health, physical activity, social capacity and vigor), showing a worse foot
and general health-related quality of life (with lower scores for all FHSQ
domains) in the women with fibromyalgia compared to healthy matched
women.
Conclusions: Impaired foot and general health-related quality of life was
observed in women who suffered from fibromyalgia compared to healthy
matched women.

Background: Fibromyalgia (FM) is a syndrome of persistent widespread pain, stiffness, fatigue, disrupted and
unrefreshing sleep, and cognitive difficulties, often accompanied by multiple other unexplained symptoms, anxiety
and/or depression, and functional impairment of activities of daily living
Objectives: To assess the frequency and pattern of enthesopathy in primary FM using musculoskeletal ultrasound
with Glasgow Ultrasound Enthesitis Scoring System (GUESS) and Correlate these findings with clinical and
laboratory parameters of FM.
Patients and Methods: A cross-sectional study includes 100 persons divided into two groups as follows: Group
(1): 50 patients with primary fibromyalgia syndrome (FM) were diagnosed. Group (2): 50 apparently healthy
controls with comparable age and sex. Patients were selected from those attending the outpatient clinic of the
Rheumatology & Rehabilitation Department of Al-Azhar University Hospital, Assiut.
Results: There is no statistically significant difference between cases and control groups regarding age, gender, and
BMI (P>0.05 for each), there were no statistically significant correlations between the GUESS scores and any of
the clinical characteristics of FM patients. Although non-significant, these correlations were negative between the
GUESS score and the tender point number, the Widespread Pain Index as well as the Fibromyalgia Impact
Questionnaire. The total GUESS score of the FM group ranged between 0-12 with a mean ±SD of 5.3 ± 2.8. The
total GUESS score of control group ranged between 0-6 with a mean ±SD of 0.97± 1.58, with a significant difference
between the two groups (P<0.05)
Conclusion: There were significant enthesopathic changes detected among FM patients especially affecting the
Achilles, quadriceps, and proximal patellar tendons.

Prevalence and Characteristics of Fibromyalgia in Patients with Foot and Ankle Pain: The Experience of an Academic Podiatry Clinic

Research about the prevalence of fibromyalgia in podiatric patients is limited, with data suggesting potentially higher estimates and greater foot impairment in patients with fibromyalgia compared to healthy individuals. The aim of our study is to assess the prevalence of fibromyalgia in the podiatric healthcare setting and to research the characteristics of fibromyalgia patients with foot or ankle disorders. Consecutive patients visiting the academic podiatry clinic at the University of Bologna IRCCS Rizzoli Orthopaedic Institute between 11 January and 31 March 2021 were enrolled. Of the 151 patients included, 21 met the fibromyalgia survey diagnostic criteria, accounting for a prevalence of 13.9% (95% CI 8.8–20.5). As part of the podiatric assessment, the Foot Function Index (FFI) was used to calculate the impact of foot and ankle problems. Moreover, patients with fibromyalgia were asked to complete the fibromyalgia impact questionnaire (FIQ). Fibromyalgia patients had significantly worse total FFI scores (63.4 ± 23.0% vs. 53.2 ± 20.3%, p = 0.038) and there was a significant linear correlation between the FFI and the FIQ (r = 0.72, p < 0.001). The prevalence of fibromyalgia in the academic podiatry clinic being 13.9% confirms that, in the healthcare setting, the disease can be more frequent than in the general population. Furthermore, our findings suggest a strong correlation between foot impairment and the impact of fibromyalgia.

Impact of fibromyalgia and related factors on foot function and quality of life: Cross-sectional study☆

Background
The aim of study is to examine the factors that may influence pain, disability and the limitation of activity due to the presence of fibromyalgia in the foot.

Methods
323 patients diagnosed with fibromyalgia were recruited. Each participant completed the Foot Function Index questionnaire (FFI) and the Revised Fibromyalgia Impact Questionnaire (FIQR). A multivariate analysis was performed to determine the factors associated with high scores in each of these questionnaires.

Results
In both questionnaires, the subscales presenting the highest scores were foot pain (FFI score: 71.18±20.40) and symptom intensity (FIQR score: 36.23±8.04). According to the multivariate analysis, foot function is influenced by age (p=<0.001), BMI (p=0.001), lack of physical activity (p=<0.001), the presence of rheumatoid arthritis (p=0.012), retirement due to disability (p=<0.001) and being unemployed (p<0.001).

Conclusion
Fibromyalgia affects foot function, provoking significant pain. Related factors include age, BMI, lack of physical activity, the presence of rheumatoid arthritis, and employment status.

Can Early Fatigue in Leg Muscles After Exercise Cause Postural Instability in Women With Fibromyalgia?

Objectives: This study aimed to investigate the effects of exercise-induced lower limb muscle fatigue on postural stability in female patients with fibromyalgia (FMS).

Method: This study included 19 female patients diagnosed with FMS according to the 2010 American College of Rheumatology criteria and 19 age-matched healthy controls. Muscle fatigue was induced by chair sit-to-stand (STS) repetitions in all participants. Postural stability was evaluated before and immediately after muscle fatigue test (MFT) in standard bipedal and tandem stances with eyes open (EO) and eyes closed (EC), and in single leg stance with EO using a foot pressure platform.

Results: The mean number of STS repetitions on MFT was lower in FMS patients (p<0.001). Before MFT, plantar center of pressure (CoP) excursions was greater in FMS patients versus controls in all postures tested except in EO bipedal stance (all p<0.01). Postural sway increased post-MFT in both FMS and control groups in all stances (all p<0.001). Post-MFT increase in postural sway was greater in the FMS group versus controls (all p<0.05).

Conclusion: Lower limb muscle fatigue occurs earlier in FMS patients than in healthy individuals and exacerbates postural stability problems. This indicates the importance of planning tailored exercise programs for these patients.
